For what ever it is worth I had muscle loss / weakness a couple of months before the rash showed up. Stiff, sore, and nasty fingernail beds were my first symptoms.

When it comes to dry skin treatments with out grease I use Amlactin. My dermatologist gave me a prescription for Ammonium lactate (Lac-hydrin) lotion. Amlactin is a over the counter version of it and it costs a little less than half as much.
