Hi, That's quite a question. All life is "genetic" so this needs some trimming. "Sporadic" IBM probably has several genetic or hereditary factors which more easily allow genes to adjust to cellular stresses. Why the adjustments lead to accumulated toxic waste probably involves predispositions which lie deep in cellular memory, jumping generations...and that is genetic and hereditary. Are there any somewhat similar neuromuscular diseases in a person's background? Myasthenia Gravis, Multiple Sclerosis....they don't have to be "our" disease. The pathways may differ and might not precipitate particles as in sIBM but add to toxic oxidative stress in other areas. This does become quite a cross-sectionally related topic. Best wishes, Del
